Jen provides Cultural Advocacy whilst Act as a bridge between patients and healthcare providers, fostering positive relationships and outcomes. She delivers clinical care such as health assessments, chronic disease management, immunisations/vaccinations and wound care. Jen provides education on nutrition, lifestyle, disease prevention, and health literacy.
Jen provides Care Coordination for patient care by liaising with multiple health services, specialists, and organisations, supporting patients in navigating complex healthcare systems, ensuring continuity of care.
