Health Assessments

Doctors and nurses can assist you to determine your eligibility for any of the following:

  • People 40-49 years of age with a high risk of developing type 2 diabetes.
  • People 45-49 years of age who are at risk of developing a chronic disease
  • People who are 75 years of age and older
  • People with an intellectual disability
  • Patients of Aboriginal and Torres Strait Islander descent
  • Please call us to discus making an appointment

Results

  • During your consultation your doctor will discuss a plan for providing your results.
  • If you receive an SMS or phone call to book in for ‘non-urgent results’ your doctor would like you to make an appointment, ideally within a couple of weeks to discuss your results.
  • If you wish to obtain copies of your results, please ask during your consultation. Alternatively, you can access these through your MyHealth record after 7-14 days.
